Youth and young adults face unique barriers when experiencing homelessness, such as being disconnected from services and lacking stable support networks. Nearly one in five unsheltered youth in Sacramento has a history of foster care before age 18, and youth often remain invisible in data due to temporary living situations. Without a plan informed by their lived experiences, our region risks increasing the incidence and duration of youth homelessness.
The Coordinated Community Plan (CCP) addresses this by providing a single, unified blueprint for change in Sacramento. Developed through six months of listening sessions and collaborative meetings, this plan identifies system gaps, defines clear strategies, and fosters cross-sector partnerships. The plan is shaped and managed by the Sacramento Youth Action Board (YAB), which has used its lived experience to identify critical gaps and guide strategic investment of federal funding.
