At the heart of Sacramento’s homelessness response is a network of public and private organizations that serve our community.

Partners and providers create an essential web of service and care.

Sacramento’s homelessness response is powered by a system of partners and providers working together to fund, coordinate, and deliver critical services. Together, they provide individuals and families in our community with a range of resources.

Anchoring this web of support is the Continuum of Care, providing federal funding for locally implemented homelessness resolution.  Next are a set of key agencies that partner to create plans, to secure funds, and manage a range of services.

Additionally, hundreds of community organizations, including independent non-profits, faith and service-oriented groups, community centers, food lockers, and others, provide services, shelter, and housing. Hospital systems, urgent care centers, and managed care plans play a critical role as essential healthcare partners. And, businesses are involved in developing and allocating housing and contributing funds.

Each of these many partners—including the numerous unlisted service providers and advocates—plays an essential role in resolving the circumstances of homelessness for Sacramento.

Lead regional partners and their roles:

Federally designated, community-driven decision-making body that coordinates funding, programs, and strategies to address homelessness.
Lead agency for the Sacramento Continuum of Care and manager of community homelessness data, plans, and systems.
Through the Department of Homelessness Services and Housing, the County provides services to those experiencing homelessness in the unincorporated areas of Sacramento.

The Department of Community Response deploys specialists to engage housed and unhoused residents and manages the City’s homelessness services.

Funded by the Continuum of Care, City of Sacramento, Sacramento County, and managed by Sacramento Steps Forward, a coordinated services system that includes hundreds of regional provider organizations.

Collaboratively managed by Sacramento County and City of Sacramento, supports affordable housing development and housing assistance programs.
