Sacramento Steps Forward (SSF) is inviting qualified organizations to apply for funding to deliver Problem-Solving Access Point (PSAP) services across Sacramento County. This initiative is part of a regional homelessness prevention strategy designed to stabilize households at imminent risk of homelessness.
Selected providers will offer housing-focused interventions, including prevention assessments, housing stabilization planning, landlord engagement and mediation, resource navigation, and coordination of financial assistance. Services will align with the Coordinated Access System (CAS) and Sacramento Community Standards for homelessness prevention.

Funding
- Total Available: $900,000 over two years across one or more organizations
- Average Annual Allocation: Approximately $75,000 per full-time position/site
- Funding Source: California Homeless Housing, Assistance, and Prevention (HHAP) Program, Rounds 4 and 5
- Contract Term: February 1, 2026 – January 31, 2028 (with option for no-cost extension through June 30, 2028)
Services to be Delivered by Prevention Providers
Prevention providers will operate PSAPs to assist households at imminent risk of homelessness. Core services include:
- 2-1-1 Appointment Response and Coordination: Respond to referrals and schedule assessments.
- Prevention Assessment and Housing Stabilization Planning: Develop individualized plans to avert shelter entry.
- Landlord Engagement and Mediation: Resolve conflicts and prevent eviction.
- Financial Assistance and Resource Navigation: Coordinate rental assistance, utility support, and other resources.
- Community-Based Access: Provide services at accessible community hubs and through mobile outreach.
- HMIS Data Entry and Reporting: Ensure timely and accurate data entry for all enrollments and services.
Proposal Evaluation Criteria
Proposals will be evaluated in three steps:
