Sacramento’s Plan to Address Homelessness

Leading change by going all in with Sacramento’s Regionally Coordinated Homelessness Action Plan (RCHAP).

Complex needs call for regional alignment.

Too often, responses to homelessness are siloed, reactive, or crisis based. Key challenges include:

Local leaders, providers, and community members knew a more coordinated approach was necessary to address these interconnected issues.

A Regionally Coordinated Framework and Action Plan

Developed in partnership among the Sacramento Continuum of Care, City of Sacramento, Sacramento County, Sacramento Housing and Redevelopment Authority, and Sacramento Steps Forward, the Regionally Coordinated Homelessness Action Plan (RCHAP) builds on the 2022 Local Homeless Action Plan and reflects input from hundreds of community members, providers, and leaders.

These plans have been integral to Sacramento’s regional application for state Homeless Housing, Assistance, and Prevention (HHAP) funding.

This collective approach:

  • Unifies efforts under one strategic framework
  • Centers key solutions to resolve homelessness
  • Frames a three-year roadmap with measurable goals
  • Embeds community voice and lived expertise in planning and implementation

This is how Sacramento creates the plan together:

Step 1:
Bring people together to define priorities

Planning meetings with partners from across the region identify needs, share insights, and refine the focus of the plan. This includes local agencies, providers, youth representatives, behavioral health staff, and people with lived expertise. 

Step 2:
Align the plan and finalize commitments

Feedback is integrated, and once aligned, the plan is formally reviewed and approved by each partner. It also fulfills a state requirement for Homeless Housing, Assistance, and Prevention (HHAP) funding, which supports critical pieces of the work outlined in the plan.

Step 3:
Take action and stay accountable

Implementation begins immediately. Partners put the plan into motion, applying HHAP and other resources to priority actions. Progress is reported through regular updates to keep the community informed and involved.

A few examples of the many “wins” from this plan.

Improved Housing Outcomes Through Case Conferencing

From February to December 2024, case conferencing supported 560 households. Nearly half exited with a positive housing outcome.

New Shelter Opened Through Regional Partnership

The Stockton Blvd. Safe Stay Community added 175 beds, supported by County and City teams and state Behavioral Health Bridge Housing funds.

Landlord Engagement Fuels Street to Housing Pilot

Two nonprofits received $5.85 million to house 100 individuals living on the parkway, connecting outreach, case management, and rental support.
