Local leaders, providers, and community members recognize that a more coordinated approach is necessary to address these interconnected issues.
In 2024, a three-year Regionally Coordinated Homelessness Action Plan (RCHAP) was developed with input from hundreds of community members, providers, and leaders. The process was led by a partnership among the Sacramento Continuum of Care, City of Sacramento, Sacramento County, Sacramento Housing and Redevelopment Authority, and Sacramento Steps Forward. It built on the 2022 Local Homeless Action Plan.
Each year the RCHAP will be reviewed and updated with community and regional leader input to ensure the Plan remains responsive to community needs and state funding guidelines.
In 2025 this review process took place during the Continuum of Care Annual Meeting and a follow-up leadership forum, and resulted in an updated plan for Sacramento. Learn more about the information presented and discussed during these sessions, and key take-aways on the Annual Meeting recap page.
Planning meetings with partners from across the region identify needs, share insights, and refine the focus of the plan. This includes local agencies, providers, youth representatives, behavioral health staff, and people with lived expertise.
Implementation begins immediately. Partners put the plan into motion, applying HHAP and other resources to priority actions. Progress is reported through regular updates to keep the community informed and involved.
From February to December 2024, case conferencing supported 560 households. Nearly half exited with a positive housing outcome.
The Stockton Blvd. Safe Stay Community added 175 beds, supported by County and City teams and state Behavioral Health Bridge Housing funds.
Two nonprofits received $5.85 million to house 100 individuals living on the parkway, connecting outreach, case management, and rental support.

Regional plans build on past efforts by aligning agencies, providers, and community across jurisdictions and throughout the Sacramento region under one action framework, with shared goals, implementation teams, and continuous monitoring.
In 2024, representatives of nearly 250 partners, including providers, agencies, elected officials, and people with lived experience, participated in creating the regional plan. In 2025, approximately 400 representatives participated.
Expanded prevention services, added shelter beds, new rapid rehousing programs, updated assessments, and increased collaboration are just a few early wins.
